Role Title: Home Visitor

Role Summary: Track patients registered on the Antiretroviral therapy and help them to adhere to the therapy. Reports to: ISS Clinic Manager Duration: 3 months (To be hired on rolling basis)

Location: Mulago ISS Clinic

Duties and responsibilities

 Prepare a list of patients who have missed clinic appointments for the MJAP supported facilities

 Make home follow-ups of patients who have not come for their clinic appointments

 Counsel patients on adhering to their clinic appointments

 Counsel patients and families on Antiretroviral therapy adherence

 Follow up on clients who are very weak for support

 Do phlebotomy and deliver medicine to patients who can’t make it to the facility

 Take history on circumstances that precede the death of patients

 At times attends to patients as a clinical services team member

 Register new patients(open up new files)

 Contact newly diagnosed TB patients whose phone contacts are off

 Take full responsibility of the program motorcycle when in the field.

Job Specifications:

 Diploma in Clinical Medicine and Community Health, Comprehensive Nursing or Registered Nursing.

 Preference is male nurses with two years’ relevant experience.

 Knowledge in laboratory quality improvement.

 Training in comprehensive HIV/AIDS management

 Excellent interpersonal, communication and counselling skills

 Very good documentation and reporting skills

 Ability to ride a motorcycle with a valid riding license (must attach a copy of the license)
