Makerere University Joint AIDS Program (MJAP) is a private-not-for profit Company under Makerere University. MJAP works to build partnerships and strengthen health systems to optimally respond to HIV/AIDS, TB, Non-communicable diseases (NCDs) and other diseases of public health importance in Africa.
MJAP is implementing a sub-award grant from Infectious Diseases Institute (IDI), for accelerating HIV/AIDS Epidemic Control in Kampala region; with funding from the President’s Emergency Plan for AIDS Relief (PEPFAR).
MJAP is seeking competent, passionate and dynamic individuals to fill the position of;
Role Title: KP/PP Peer Linkage Facilitator
Reports to: Community Linkage/Prevention Coordinator
Location: Makerere University Hospital & Butabika NRH
Role Definition: To mobilise, track & follow up HIV negative clients on PrEP (retention), follow up KPs/PPs lost from care in MJAP supported sites, support effective linkage of all HIV positive KPs/PPs identified from the community to MJAP supported care centres or as otherwise mutually agreed
Key Result Areas: Improved KP/PP care, adolescent care, treatment and support services at the supported site (s)
Key roles and responsibilities
1) Linking KPs/PPs to health facilities and any other community where they need support.
2) Give health education on key HIV prevention interventions
3) Mobilisation of KPs/PPs for uptake of services in both the outreaches and supported health facilities
4) Drug & condom distribution
5) Making PrEP refills
6) Making reports on PrEP clients
7) Making prompt follow-ups of KPs/PPs who miss their appointments
8) Providing psychosocial support to fellow KP/PP peers on drug adherence, combination prevention packages and stigma reduction
9) Encouraging KPs/PPs to remain in care and ensure good retention at the supported facilities
10) Identifying KPs/PPs in communities who need services and linking them to health facilities
11) Documentation of the gathered data in the relevant tools
12) Compilation of summary reports to the supervisor
Qualifications & job specifications:
Age 20 years and above
At least UCE certificate
Special knowledge in the management of HIV/AIDS and its related complications, confidentiality, team player, ability to empathize and be patient and counseling skills.
